This is followed by a sharp dissection, undermining the subpectoral space, cutting the muscle’s insertion medially from the sternum up to 4–5 cm from the mammary fold. Inferiorly, dissection is completed achieving the inframammary fold after complete severance of the pectoralis ﬁbbers. The lateral extent of the dissection continues beneath the serratus anterior muscle, and stops at the predetermined lateral border of the breast (Figs. 2 and 3). If the lateral skin of the mastectomy is thick and well irrigated, a complete muscular pocket can be avoided, in order to give more lateral projection to the breast.
